Patient Forms

If you are a new patient to our practice, please print and fill out our New Patient form and HIPAA Privacy form and bring them with you to your first visit. This will save you time when you arrive.

*Lakeway Eye Physicians and Surgeons accepts most major insurance plans. Please call our office at 512-402-9919 to see if we accept yours! Lakeway Eye Physicians and Surgeons will file your insurance claim as a courtesy to you. There is no guarantee that payment will be made for services rendered. If you need a referral from your Primary Care Physician, it is your responsibility to obtain one.

If you are looking for a good ophthalmologist in the Lakeway area, I would like to recommend Corina Stancey- 402-9919. I had a scratch on my cornea that developed into a serious eye infection. I notified her on a Sunday afternoon as she was doing some Christmas shopping. She left the mall to meet me at her office to look at my eye. She then saw me for the next 3 days and made it possible for my husband and I to leave on the 4th day to take a cruise we had scheduled over the Christmas/New Year’s holidays. If it had not been for Dr. Stancey, we would have had to cancel our trip. She also gave me recommendations of another ophthalmologist in San Juan in case I needed one and she kept in touch with me via email to see how things were going. She is a very caring person and I recommend her very highly.
